Circle Health is a unified browser extension that integrates with MDland/iClinic, Practice Fusion, eClinicalWorks (eCW), and Elation EHR platforms.
It streamlines patient enrollment into Circle Health's Chronic Care Management (CCM) program by displaying an enrollment panel for eligible patients with at least two ICD codes. Healthcare professionals can select relevant ICD codes and enroll patients directly from supported EHR platforms.
The extension supports secure authentication through Circle Health's OTP login and provides a legacy API token option for compatible platforms. It also includes session status indicators and token management to simplify access to Circle Health services.
