The Enhanced Middle Click won't work properly if you have "use autoscrolling" enabled in your preferences. You must first disable it in Preferences > Advanced > General > Use autoscrolling.

In version 0.4.6 AutoScrolling action was removed due to undocumented changes in Firefox API regarding executing AutoScroll. See on SO
Activating context menu on bookmarked item (not folders) in bookmarks menus will display context menu for a short time then it will open the bookmark. Simple workaround is to not release the right mouse button upon activating context menu and just move mouse over the context menu item. Hopefully this will be fixed in the next release.

On linux middle click is used to paste selection buffer. If you have an URL in your selection buffer, firefox might try to open it on middle click. A quick fix is to select some normal text. A longterm fix is to disable middlemouse.contentLoadURL in about:config.
