I'm not sure what to make of this... When my Google search results contains a link to Apple or Amazon, this add-on insert an image of the logo of Apple or Amazon. The link associated to this image is the same as the one provided by Google, but the image itself is located in an 'affiliates' folder on smartestbookmarksbar.com.
Maybe the author is trying to see if he could make money with this?
In reply to Paul Young's comment below: The web site you're visiting needs to have alternate style sheets for this add-in to be useful. It duplicates the View, Page Style menu of Firefox in the context menu of the page for quicker access.This review is for a previous version of the add-on (1.1.2a).
