I would give 5 stars except for a few issues listed below, I couldnt find a dev website in order to post them there :(
First, I have the tabs on the left of the browser, however when this is the case, the close tab button is on the left of of the tab which can cause you to close the tab in error. It would be better if the close tab button was on the right, with the webpage icon on the left.
Second. The tab loading animation appears to go backwards, ie it goes from right to left, not left to right when the bar is on the left.
Third. The justification seems to be wrong when on the left. It indents from the right rather than the left, leaving short urls/descriptions on the right hand side not justified to the left.
I suspect it's because it is just a flipped version of the right hand tabbed section?
If these were fixed, it would be perfect. As well as the problem mentioned by Shingosi regarding tabs that extend beyond the bottom of the tabbed area. There is no way to access them, though I would rarely have so many tabs open in reality.
Answer to your remarks
Your first issue is interesting; I had thought that the farthest from the page were the close buttons, the less likely was a mis-click (not to mention Right-To-Left users). I can try to provide a preference to change that too.
The label align is configurable in the VertTabbar options, just check "reverse align of tab title" (or the translation of that in your language); I wouldn't be surprised if this changes also the loading animation direction.
Lastly, the tab problem is really a problem with TabMixPlus, which assumes the bar is horizontal. With a vanilla Firefox, the scrolling is normally handled.
The only way to fix that would be to patch a big chunk of TabMixPlus from my addon, which means a lot of compatibility problems with newer versions, and I'm wary of diving into this extension hell.

THIS SITUATION REMAINS TRUE!! NOT FIXED. 2009.03.18
There seems to be no way to scroll the tabs beyond those that are shown. The scroll button that was visible without this extension, is no longer present. I currently have 112 tabs open, and frequently switch between them. It seems the number of viewable tabs are limited to about 29-30 (at 1280x1024 resolution). Once you go beyond that number of tabs, forget about seeing the rest of your tabs.
There is scroll capacity
The problem comes from TabMixPlus, which cannot handle vertical scrolling in the tab bar, and doesn't detect there are too much tabs. Without TabMixPlus, the tab bar correctly adds scroll arrows when necessary. That said, the "all tabs" menu of TabMixPlus still works perfectly (which is less than ideal but at least you can access your tabs).

Cool idea... but i remove it. I want the bar in to the left side, but this is not possible!This review is for a previous version of the add-on (2.5).
It should be possible
Does modifying the setting in VertTabbar options work ? Maybe there's a bug here.

Can't believe I'm giving 4 stars to something I'm not going to use, but... it's almost perfect - can even be resized! The longest raw way to resize is: Tools / Add-Ons / VertTabbar / Options. Only one MAJOR problem - on mouse-over, the size of EVERYTHING changes, and when you mouse-over something else, seems like the entire computer is shaking. If only it would highlight peacefully, it would be perfect! :( For now, I'll disable, wait for updates and hope hope HOPE that an updated version has normal mouse-over function.This review is for a previous version of the add-on (2.5).
What you describe is not supposed to happen; could you send me an email with the extensions you have, and/or try with others extensions disabled ? It may be an incompatibility with some settings of Firefox, or with another ext, and I'd like to fix the bug either way.

its small and simple, just what i wanted, but it does not work with TabMixPlus, and most of us want both extensions i think...This review is for a previous version of the add-on (2.4).

I am using this as Tree Style Tab isn't working in the latest nightlies.
Some things I think it lacks are the ability to re-order your tabs by dragging them up/down and you can't drag to resize it, you have to open the options and set the width. This can be a pain but it's easy to see why you would want that as accidental resizing of the bar can be just as annoying. A neat extension that gets the job done and helps me reclaim my screen space. Excellent.

Is there a plan to add the ability to change the relative position of the tabs?
For example, I always keep my mail as the top tab but if I open it second I can't move it up there..unless I am missing something
