Nevermind, i've figured it out.
This is a great add-on that shouldn't exist :). What I mean is, it shouldn't have required me to use an add-on to make my tools menu available in the little orange menu.
This is what I would like to see in this yet:
1) an option to make toolbar boxes layout vertical instead of horizontal
2) I would also like to see it possible to remove the "Edit this menu..." item to make my menu even simpler, making it available using a right-click instead (ideally, as part of the customize... toolbar menu).
3) Split the special "Bookmark" and "History" menu buttons into a different add-on. I want this add-on to customize the menu, which already has a bookmark and history items in it. There existence doesn't seem to have anything to do with that.

I'd like to be able to put my bookmark toolbar items on the menu (I don't have a lot). However, after reading the "developer notes" above, and following the directions in the first item, I don't see a "Bookmarks Toolbar Items" on the available list. I see a "Bookmarks Toolbar", but all that does is create a single icon that shows the bookmarks in a submenu.

The group of bookmark icons at bottom-left, is "Bookmarks Toolbar Items", inside a Toolbar Box. If you want to add this one, you need to remove it from Bookmarks Toolbar via "View -> Toolbars -> Customize". Then go to Personal Menu Options, add an available item called "Toolbar Box", finally you may see the "Bookmarks Toolbar Items" in the available item list."
