Just installed this extension, and it seems to do what it says. However, in the case of animated gifs, the rotated picture will no longer be animated, showing only the first frame.
I was on FF 18.0.2. Addons page said Imagetwist wouldn't work with my version. I upgraded FF to version 22.0. After the upgrade it would allow me to install Imagetwist, and the addon manager said it was installed *and* enabled, but I had no rotate option in the right click menu. And not in any other menus. Attempts to view rotated images didn't result in them being correctly oriented. Removed and installed a 2nd time to no effect. I have a large number of other extensions installed, could be a conflict, but none of those other extensions deal with images except for "Save Images 0.9.4".

This is so good I'm retiring my "Flip or Rotate Image" extension (and pointing people that find it to here), which used the CSS transform method you cleverly avoided, and doesn't have the brilliant EXIF detection of yours.
If you added a transition/animation of the rotation and made the menu show up in Private Browsing windows I'd be completely done :)
Thanks for the kind words :) I'll file both of the improvements you mentioned as Issues on github. The private browsing one is probably trivial, and the transition would be a neat twist I'd be happy to add.
Works just as expected.
This works fine in FF21, but where I really need it is in Thunderbird. iphone images are often on their side. So far, no luck in hacking the install.rdf and TB trunk.
