I have tested the AddOn several times on different machines on Windows 10 and Windows 7 in English and German.
This AddOn works up to the latest nightly build on FF32 and FF64. So please check your places database. Perhaps you use a script blocker or something else.
You might test a clean portable version.
You might export your bookmarks, delete them and import them again.
There is another user who tested other AddOns for sorting bookmarks. Perhaps those AddOns destroys the places database of FF.

Was working just fine a couple weeks ago. Nothing at all happens when I click on it now (either from the bookmarks menu, or from the context menu). Only thing that has changed is update to FF 45.0.1. No, no new add-ons. Running under fully-patched Win 8.1. FavIcon Reloader 0.8.1-signed. ..... Note that 5-star rating reflects prior experience ...! Please fix ... thanks ...!
I don't have a portable FF to test with. However, with FF 43.0.1, I created a brand new profile, and installed ONLY FavIcon Reloader. Both before and after importing my bookmarks, when I clicked on the Reload Favorite Icons menu entry, the Reload Favorite Icons window would come up, but there were NO entries in it at all. Also, it took on the order of a minute or so for that empty window to open (it used to only take a few seconds to open). Further, when I checkmarked the "show debug text in extra column" option, the Reload Favorite Icons window would no longer open, at all.
And let me repeat ... the app worked just fine until I upgraded FF to 45.0.1, then it quit working ... WITH NO CHANGES TO ANY ADD-ONS WHATSOEVER. Hope the above related behaviors will help you to find the problem with the app!

Working again - I didn't do anything different just disabled Windows Defender for 2 mins.
Upgraded to FF45.
This GREAT tool is no longer working.
It did crash FF two times.
FF 32 bit and 64 bit not working on both.
Disabled all add-ons, still not working.
No windows comes up anymore when i start the reload.
On Windows10, nothing happens.

I have a large amount of bookmarks and this addon reloaded all the favicons for more after I had to do a profile reset. It worked quite well.

(This was done on a brand new install of Windows 7 x64 and Firefox 44.0.2.)
Importing from a bookmark backup makes all favicons disappear, so this addon was exactly what I needed. However, it crash firefox shortly after starting. I thought it was because I had a lot of bookmarks, but reading the reviews says that shouldn't be an issue, so I ran it again to see what happened. It crashed again, and I ran it again. It crashed, I started it again. With each start, I noticed that all the links that had had their favicon resorted weren't showing up and it was as if the program was going "from where it left off". It took four or five crashes before it made it to the end.
So, yes, it will reload the favicons and it will work with a large amount of bookmarks, but it shouldn't be crashing firefox and shouldn't have to be restarted in order to do so. Had I not played with it as I did, which I believe most people would not, I would have quickly just tossed it off as a bad addon.
I'm leaving this review so others will have some reference if they encounter a similar issue. Also, I highly doubt it is supposed to work this way, so hopefully the developer can figure out what's going on.
Response to Developer: I mean crashes exactly like I said, it crashes Firefox completely and brings up the "Mozilla Crash Reporter". I don't think that counts as a "script error", but I could be wrong.

There are no other reports of crashing issues.
So I thing there might be an other AddOn which causes this behaviour. Please test with deactivated AddOns.

Switched to new profile, synced my bookmarks and lost all favicons. But thanks to this extension i can reload most of them! I even can see the progress and have an option to pause it! Cool!

This extension works perfectly. No frills, it does what it says and it does it well. I have literally hundreds of bookmarks and it churned through them with no problems at all.

More options would be good, such as a "right click" option that allows you to add the favicon from another source, or something like that. Many favorites links do not load a favicon because there doesn't seem to be one.
within my private beta version there are some changes but this will never be released.
This AddOn becomes obsolete in late 2016.

Doesn't have About-Page, which causes FF (recent) to freeze/"become inaccessable" when calling about. Doesn't have a Link to This- or it's Home-Page. If there even is any, anyhow anyways.

I restored my bookmarks from another computer after sync made a mess on this one, and lost all the favicons. Favicon Reloader took 15 minutes to reload 4000 bookmarks. It used 1300MB of memory on top of base 500MB.

I only wish for the missing feature of exceptions, I got a site with limited views per day and a ton of bookmarks, this one could get you banned from those sites for mistacking it for a site ripper.
Another more efficient solution will make this faster using less bandwith and less memory but more code would be to load favicons per domain, so if you have 50 www.deviantart.com/*** booksmarks it will only need to load the first one then copy it to the 49 bookmarks.

Addon is too old. Good, but not updated.
OK I tested this. It works when you do not have a lot of bookmarks. Work on all FF versions. I have many, many bookmarks and addon not see my bookmarks completely. Addon cause high memory usage, if we have a lot of bookmarks(there may not be too much). I recommended Firemin beta program, then firefox not crash. Author addon only reply to users and do not fix this addon. Author not test this addon completely on latest FF versions, with many bookmarks...

Not develop for special users? Author this addon not update and write this? This is so funny. A lot of addons work on beta/stable/aurora/nightly. By author addon, addons do not work already on the Firefox beta ? Hm... Author instead of replying to comments, let this add-on update!
Tis AddOn is well tested
If you look through the reviews you'll find one where I have tested a case with 200.000 Bookmarks.
This FF AddOn is not compatible with new Beta Versions of FF and it will not be compatible because of a changed API.
This new API will not support low level calls to the Bookmark and FavIcon handling. This has nothing to do with "the author will not update..." It's simply not possible.
If you just have AddOns that changes the DOM or analyses web pages, that's not a problem. But FavIconReloader needs access to the inner places structure of FF and this is not possible anymore. In addition the new API does not allow mass screen updates because of using html and disallow XUL.
In addition there is no support for real multi tasking.
So if you're low on knowledge you should not demand changes or otherwise the author has to delete his AddOn. Clearly said: Please don't use my AddOn instead of being so fussy.
This AddOn will be available to all users who uses ESR Versions of FF.
The rest of the users should use an other AddOn (oh wait, there aren't another one) to fill up their FavIcons.
I don't want to have discussions with those users as above.
